Rheumatoid arthritis can begin subtly, before any visible deformity develops. Recognizing its features can lead to earlier assessment. Current treatments can reduce disease activity and limit joint damage, particularly when care begins promptly. [1, 2]

01 / Overview

More than joint pain

Rheumatoid arthritis, or RA, is a chronic autoimmune disease. The immune system drives inflammation in the synovial membrane lining the joints. Over time, this inflammation may damage cartilage, bone and the structures that stabilize a joint. RA is different from osteoarthritis, although a person can have both conditions. [1, 3]

Several factors contribute to its development. Genetic susceptibility, female sex and smoking are associated with RA risk; obesity may also play a role. These associations cannot predict who will develop the disease and do not establish a diagnosis. [3, 4]

02 / Symptoms

Recognizing symptoms before deformities develop

The small joints of the hands and feet, together with the wrists, are commonly affected. Symptoms often occur on both sides, although exact symmetry is not required. Joints may feel swollen, warm, tender and harder to move. Stiffness after rest, especially on waking, often lasts more than 30 minutes. Fatigue and sometimes a mild fever may accompany the joint symptoms. [5]

Finger drift and deformities such as swan-neck changes illustrate established disease. They are not required for diagnosis and are not inevitable. The hand shown on the poster is an illustration of an established presentation, not a representation of every person with RA. [1, 3]

03 / Diagnosis

Diagnosis brings together several findings

Assessment combines the symptom history, joint examination, blood tests and, when appropriate, imaging. Rheumatoid factor and anti-CCP antibodies can support the diagnosis. C-reactive protein (CRP) and the erythrocyte sedimentation rate (ESR) provide information about inflammation, while a blood count may identify anaemia. No single test can confirm or exclude RA on its own. [6, 7]

X-rays, ultrasound or sometimes MRI may add useful information. Normal blood tests should not delay referral when persistent joint swelling suggests inflammatory arthritis, particularly if several joints or the small joints of the hands and feet are involved. [6, 8]

04 / Treatment

Controlling inflammation and preserving function

Disease-modifying antirheumatic drugs, or DMARDs, aim to control the inflammatory process and limit damage. Methotrexate is commonly used. Other conventional DMARDs, including hydroxychloroquine, sulfasalazine or leflunomide, may be selected according to the individual situation. Biologic and targeted treatments have specific indications and require an individual assessment of benefits and risks. [9, 10]

Anti-inflammatory medicines can ease pain and stiffness but do not replace a DMARD. Corticosteroids may be prescribed in selected situations, usually for a limited time. Rheumatology care aims for remission or low disease activity and adjusts treatment according to response and tolerability. Surgery is sometimes needed for significant joint damage. [9, 10]

05 / Physical activity

Staying active with an appropriate programme

Regular, adapted exercise is recommended. A programme may combine mobility, strengthening and aerobic activity while accounting for affected joints and flares. Physiotherapy supports movement; occupational therapy can offer ways to protect joints, adapt everyday tasks and maintain independence. These measures complement medical treatment. [11]

It is also useful to discuss smoking cessation, sleep, fatigue and difficulties at work or at home with the care team. A practical goal, such as fastening a shirt more easily or walking more comfortably, connects follow-up care with activities that matter to the individual. [6, 12]

06 / Follow-up

Looking beyond the hands

RA can involve nodules, eye dryness or inflammation and, in some people, lung or heart problems. Follow-up also considers cardiovascular health, bone health and infections. Infection risk may be affected by both the disease and certain treatments. Not everyone develops these complications. [6, 13]

The cervical spine may also be involved. New neck pain accompanied by weakness, numbness or difficulty walking needs prompt assessment. Any treatment involving the neck must take possible cervical instability into account. [11, 13]

07 / When to seek care

When to seek prompt assessment

Persistent joint swelling warrants medical assessment without waiting for deformities to appear. A joint that suddenly becomes very painful, hot and swollen requires urgent assessment, particularly with fever or feeling unwell, because a joint infection may be responsible. [8, 14]

Before the appointment, record when symptoms started, which joints are affected, how long morning stiffness lasts and which activities have become difficult. Bring a medication list and previous test results. Photographs taken when swelling is visible may also help explain how symptoms have changed.

The first goal is an accurate diagnosis, followed by a care plan that controls inflammation and supports everyday life.

FAQ

Frequently asked questions

<details>

Is rheumatoid arthritis the same as osteoarthritis?

No. Rheumatoid arthritis is an inflammatory autoimmune disease.
